Two supplements have clear, specific paediatric recommendations for this age group. Almost everything else in the toddler vitamin aisle is aimed at parental anxiety rather than a nutritional gap.

Vitamin D: the universal recommendation
This is the clearest guidance in infant nutrition, and it applies to essentially every baby.
The AAP’s position is that all infants receive a vitamin D supplement, with one exception covered in the next section. The doses:

The reason is straightforward: breast milk is low in vitamin D. The 27-ounce formula exception
There is one situation where a vitamin D supplement is not needed, and the threshold is specific.
The exception applies to babies consuming more than 27 ounces per day of commercial formula that has vitamin D added. Infant formula is fortified, so a baby drinking that much is already getting the recommended amount.
Two practical consequences follow, and both catch people out:
- Combination feeding usually still needs the supplement. A baby who takes some breast milk and some formula may well be under 27 ounces of formula. Do the arithmetic rather than assuming.
- Intake changes. A baby taking 30 ounces at four months may be taking far less at ten months as solids increase. The exception can quietly stop applying.

The recommendation covers infants who are partially or completely breastfeeding. It’s easy to assume that a baby having some formula is therefore covered, but the guidance doesn’t draw that line — and combination-fed babies are exactly the group most likely to fall between two sets of assumptions. Raise it with your paediatrician at the four-month visit rather than working it out alone; the dose is weight-based, which is another reason it needs a clinician rather than a label.
Note also the endpoint: supplementation continues until iron-containing complementary foods are introduced. It’s a bridge, not a permanent fixture. Iron-fortified cereals, well-cooked meat, beans and lentils take over the job.
When iron starts earlier
For some babies the four-month starting point moves forward. Iron supplementation may begin in the first month where there were complications including:
- Maternal diabetes
- Low birth weight
- Prematurity
- Small for gestational age
These all affect how much iron a baby had the chance to store before birth. If any applies to your baby, this is a conversation to have early rather than at four months — and one your paediatrician will usually raise first.
The 12-month screening
This one is easy to miss, and it’s the safety net for everything above.

All babies — not just those with symptoms, not just those who were breastfed, not just those with risk factors. Here is the most common way a well-fed toddler ends up iron deficient, and it’s entirely counter-intuitive because it happens through a food everyone considers healthy.

Case reports of severe iron-deficiency anaemia from excessive cow’s milk intake exist in the medical literature, and the children involved were not neglected — they were loved by parents who believed milk was the healthiest thing available.

Why not gummies
For this age group specifically, the objection comes before any argument about nutrition.
Gummies are a choking hazard for toddlers. They’re firm, sticky, exactly the wrong size, and designed to be chewed by children with a full set of teeth and mature swallowing. Check the age on any gummy product — most are labelled for older children for this reason.
Beyond choking, the quality problems documented across the gummy category apply here too, and matter more in a small body:
- Label accuracy. Independent testing has found gummies containing substantially more or less than stated — including a 2019 study identifying a children’s brand with 1,000% more vitamin B6 than other brands.
- Overshoot. A 2023 independent review found gummies with nearly double their claimed folic acid, approaching the tolerable upper limit.
- Serving-size variation. Serving sizes ranged from 4 to 15 gummies across brands of children’s omega-3 — so switching brands can change the dose several-fold with no signal that anything happened.
- They look like sweets. That’s the design intent, and it drives accidental overconsumption — a real risk with fat-soluble vitamins A and D, which accumulate.
For under-2s, use drops. They’re measurable, they’re the format paediatric dosing assumes, and they don’t invite a toddler to eat the bottle.
The iron warning that matters most

Does a toddler need a multivitamin?
